Disguised as an Italian medic, Dolas finds himself on a ship evacuating wounded Axis soldiers to Italy. I denne fjerde og sidste del af den hæmningsløse trilogi, går det løs med stjerner som Anna Faris (Scary Movie 1, 2 & 3, Lost In Translation), Regina Hall (Scary Movie 1, 2 & 3) og Craig Bierko (Cinderella Man) i hovedrollerne, samt fantastiske gæstespil af Carmen Electra, Michael Madsen og Leslie Nielsen. Men The Grudge er ikke stærk nok, The Village er ikke sikker nok, og Saw er ikke skarp nok, til at undgå endnu en latterkrampe i det gode gys’ navn i Scary Movie 4!
